What happens in a nutrition consult?
You will be emailed or mailed a nutrition questionnaire and a food record form that you will bring to your first visit. You can also bring all you're the medications and supplements that you take as well. Your overall medical profile, diet and lifestyle will reviewed and recommendations given.

Follow-ups are determined individually based on what you need to support your success. The length of the follow-up may vary. Follow-up by phone is also possible.
